A Seamless Onboarding Experience
Initial Call
With caring experts available whenever you call. During this first conversation, we listen closely to your needs, answer your questions, and share insights on creating a well-informed care plan. We appreciate that coordinating care can feel overwhelming, so we make it our mission to provide clear, compassionate guidance every step of the way. Following the discussion, we'll send you an information pack detailing our extensive services. Once you’re ready, we aim to have your care package in place within 3 to 7 days.
Meeting the Family
Next, we invite you to a face-to-face meeting—either at our Mantebre Care branch or, ideally, at your home. This meeting is a valuable opportunity to build a genuine connection, assess your loved one’s needs, and refine how we can best support your family. During this visit, our dedicated Care Manager discusses the proposed care plan in-depth and ensures every question is answered, with the option to schedule follow-up sessions as needed.
Personalized Care Planning
When your family agrees to move forward, we create a custom Care and Support Plan designed specifically for your loved one. This dynamic plan sets out every detail of the care we provide and is regularly reviewed and updated to reflect any changing needs. Our comprehensive plan includes:
One-Page Client Profile
The Care Support Plan
Risk Assessment (covering both the client and their environment)
Medication Risk Assessment
Your dedicated Care Manager collaborates continuously with the client, carer, and family to keep the plan current and effective.
Thoughtful Carer Matching
We believe that the right connection makes all the difference in care. Our matching process goes beyond simply assigning a caregiver; we consider your loved one’s personality, preferences, and specific needs. For smaller care packages, one or two dedicated carers may be assigned, while larger packages might involve a carefully coordinated team of up to six carers. This consistency in caring relationships helps build trust and familiarity over time.
Carer Introduction & Care Package Launch
Once the Care Support Plan is finalized, we arrange a warm, introductory meeting at your home. Here, your dedicated carer(s), client, and family come together with our Care Manager present to ensure everyone is comfortable and aligned. Following this introduction, your scheduled visits commence, and after each visit, our Oncare system provides detailed updates so you can keep track of every interaction. Your Care Manager remains in regular contact, managing any adjustments and ensuring the ongoing success of the care plan with yourself or Family member having accessing to your plan on his or her phone app.

On the rare occasion that either you or your loved one feels the carer match isn’t quite right, our Care Manager is committed to working with you to explore alternative arrangements until the perfect fit is found.
